The inactivity rule is automatic on all of the ladders. If you don't play for 2 days you get dropped. This actually helps avoid problems for those that want to try climbing the ladder and really isn't a penalty. If you were number 3 and didn't play for 4 days you shouldn't expect to come back and be number 3. The rule says there are no exceptions.

All rules in the section titled "Top Ten", excluding Inactivity, will apply to the Top Five (5) players.

Top Ten
The purpose of the Ladder is to climb to the top and the competition for the rungs in the Top Ten is high. As such, we have special rules that apply for members in the Top Ten.
Inactivity
Members in the Top Ten must play a Ladder match at least once every 2 day(s); otherwise, you will be automatically dropped five rungs and your inactivity counter is reset. There are no exceptions to this rule.
